How do I request a change to my current policy?

To make a change to your current policy, log in to your AANA account and select Make a Change from the left-hand navigation.

You have the ability to request the following changes:

  • Mailing address
  • Location of practice
  • Policy type
  • Change limits of liability
  • Cancellation

Please note: You will need to download and print the change forms. Once you have completed the form, please email it to or fax it to 800-547-2220.

The effective date of the change will be the date AANA Insurance Services receives your request or a future date you’ve requested.
